For your convenience, you may complete form below and automatically submit the application OR you may mail it to: Adopt A Spot Program~ Perry Township , 3111 Hilton Street NW, Massillon, Ohio or hand deliver the completed application to the Administration office.

After  your completed application is received, we will contact you directly to meet with you at your selected site location  to finalize your location and discuss the details of your planting plan, tree or bench memorials.

The preferred plantings are hearty ornamental trees, pampas grass, and perennials.
